以西马沉香为材，雕方形牌饰。此牌正面具有一定的弧面，此类形制对于作者雕工的要求较高。相对于平面雕刻，在弧面上雕刻难度更大。沉香牌正面浮雕四臂观音，端坐于帷幔中的莲花座上，法相庄严，周身饰有璎珞等。牌头雕饰大面积朵云纹，底边亦以朵云纹装饰。背面阴刻夔龙纹及回纹，另浮雕“平安”二字，寓意吉祥。
