此造像为八臂十一面观音象，分五层排列，正面最顶层为阿弥陀佛头像，是为修大乘的众生所作的说法相，其下四层面相各异，也是分别为不同类众生说法而显现的面相，正面二手合十，右边三手分别持念珠、法轮、施无畏印，左边三手执莲花、弓箭、净水瓶，部分法器已失，衣纹流畅美观，裙边刻有精美花纹，全身有项圈、长链、耳环、手镯、臂钏等极富装饰性，双脚并立于覆莲座上，莲花座造型宽大，做工精美，主面头戴五叶冠饰，面相方圆，表情安详，展现了观音菩萨救度众生的慈悲与关爱，在现存造像之中十分罕见。
